International Mission Team Application

This is a two page application that will take approximately 20-30 minutes to complete.
If you have a passport, you will need your passport information and a picture of the passport identification page to complete page one.
If applicable, you will need to list medical conditions and medication information on the form.
Upon completion of the first page, you will have the option to "Save & Finish Later" to complete the second page at a later date.
If you choose to finish later, you will receive an email with a link to finish your application. If left incomplete, the application will expire in 10 days and you will need to start over.

As part of our commitment to ensuring a safe and secure environment for all participants and communities involved in our 5+5 mission trips, we require all participants to undergo a background check. This helps us maintain a high standard of safety, trust, and integrity. The background check will be handled in accordance with applicable laws and regulations and will be used exclusively for the purpose of evaluating your suitability for participation in the mission trip.

Some countries require immunization and/or medication to be taken in order to enter their country. If the country does require, you must comply in order to travel to the country and participate in the mission trip. CCC is not responsible to research information, set an appointment or pay for vaccinations/medicine. It is solely your responsibility. A link to the CDC will be provided in a confirmation email after submission of application.
